Summary

Epsom and St Helier University Hospitals NHS Trust, formed in 1991, provides a range of medical services to approximately 420,000 people living across southwest London and northeast Surrey. In addition, they provide more specialist services, in particular renal and neonatal intensive care, to a wider area, covering parts of Sussex and Hampshire.

There is a unique opportunity to appoint 2 whole-time or maximum part-time Consultants in Obstetrics & Gynaecology. They are replacement posts and will join the 3 existing substantive Consultants with plans underway for a new and additional post to make a total complement of 6.

Along with Consultant colleagues you will assist in the provision of a comprehensive Obstetrics and Gynaecology service primarily to the people of South Lincolnshire.

Each of these posts will attract 10 PA’s.

A successful appointee who elects to undertake a maximum part-time contract will be required to devote substantially his/her time to the duties of the post and to give them priority at all times. In such instances modification to the job description will be discussed on a personal basis, in consultation with the Clinical Director and Consultant colleagues.

GENERAL INFORMATION

South Lincolnshire extends some 50 miles from East - West and 40 miles from North - South. It has a scattered rural population of approximately 292,300.

Our client is situated within an attractive, historical market town with a population of approximately 30,000. It is situated on the River Witham, close to the edge of the Wash and the Lincolnshire Wolds.

There is a wide variety of cultural and sporting facilities available and local schools provide a high standard of education.

Access to A1 motorway and major rail links takes a little less than one hour, via Grantham or Peterborough, providing easy access to London and the North.
Nearby international airports are the East Midlands and Humberside Airports, approximately 1.5 hours away.

The post is based within a modern district general hospital, providing a range of hospital services to a catchment population of 177,500 rising to approximately 250,000 during the summer, because of the holiday influx to Skegness and related resorts. The hospital has a total bed compliment of 640.

The maternity unit has 32 ante-natal and post-natal beds and a Special Care Baby Unit which is on the same floor as the Paediatric Unit. It has 13 cots. The delivery rate has been steadily rising in recent years and is now approximately 2,200 births per year.

THE DEPARTMENT

The Obstetrics and Gynaecology department is part of the Women’s and Children’s directorate.

A full and comprehensive range of services are available at the two main hospital sites. There is a midwifery Managed Maternity Unit as well as Ante-Natal and Gynaecology Outpatient Clinics with Day Case Surgery. Out-Reach Clinics and Day Case Surgery are also performed in Louth and Gainsborough. Out-Patient Clinics are undertaken in Skegness, Spalding and Sleaford.

DUTIES OF THE POSTS

POST 1

The successful candidate will provide leadership for the Termination of Pregnancy service and the Early Pregnancy Assessment Unit which are mostly nurse-led. The lead is expected to play an active part in ensuring that pathways and guidelines are updated and participates surgically as provided for in the job plan. Ultrasound scanning skills are essential. Ability to set up and run specialist clinics in Community Gynaecology and offer college recognised practical training in family planning will be welcomed.

In addition there are additional commitments for ante-natal clinics as well general gynaecology as indicated in the outlined job plan

POST 2

The successful appointee to this post is to expected to provide an External Cephalic Version service. It is also envisaged that the successful appointee will help to develop ambulatory gynaecology services in addition to offering a generalist practice as outlined in the job plan. Skills in Colposcopy, Operative Hysteroscopy or other expertise in out-patient and day case procedures will be particularly welcomed. This individual will assume the lead role and develop a structured rolling audit programme for the department.

The job plans are designed to complement the existing services currently provided and any change will require consultation and the agreement of colleagues. Our client is situated within the beautiful Lincolnshire countryside. It is a large purpose built modern District General Hospital, which provides a wide range of specialist clinical services to a catchment population of 450,000. It is the largest hospital within the Trust. It has a complement of 705 beds.

Introduction:

The post is intended to provide training in acute paediatrics and is particularly suitable for anyone wishing to pursue a career in hospital paediatrics, community paediatrics or general practice.

During the period wide basic paediatric experience would be gained. There is a weekly Paediatric case presentation meeting and a teaching session. There are regular Audit Meetings held within the Department and jointly with the Accident and Emergency Department and the Obstetricians.

The Hospital Paediatric Department consists of two Paediatric Wards, purpose-built Paediatric Out-Patient Department and Special Care Baby Unit.

The candidate also will gain wide basic neonatal experience. Our client is a Level Two Neonatal Unit providing Special & Intensive care from 28 weeks gestation. The Neonatal Unit is situated in the Maternity Wing on the next floor to the Delivery Suite. The delivery rate is about 3500 per year. Extremely premature infants are transferred to regional centres. Currently there are 17 Special Care and 3 Intensive Care Cots.

DUTIES OF THE POST:

1 GENERAL PAEDIATRICS:

The in-patient service is based on two children’s wards accepting children from a mixture of specialities. During the period of 6 months, it is expected that the ST1/2’s will further their experience of all the common paediatric emergencies. The ST1/2 may be called to Accident & Emergency to give advice or deal with emergencies. The Out-Patient Department is based at the County Hospital and the post holder will be expected to attend Out-Patients regularly. These sessions include Hospital based and visiting Consultants. The post holder would also be attending ambulatory unit at a nearby Hospital regularly.

2 NEONATAL PAEDIATRICS:

The SHO will be called to attend problem deliveries and examine newborns on the postnatal wards. The unit currently carries out most neonatal Intensive Care. Infants with surgical problems and extreme prematurity are transferred to sub regional centres. Further experience of practical procedures will be gained.

3 TEACHING:

Medical students are regularly attached to the Unit and the SHO’s assist with teaching including the supervision of clerking of admissions on the ward. The SHO’s are occasionally involved in teaching Nursery Nurses, Student Nurses and Midwives.

CONDITIONS OF SERVICE

There are a further 6 junior Paediatric ST1-2s who work a 1 in 7 full shift rota with prospective cover including this post. The ‘on call’ doctor is responsible for all child admissions and covers the Special Care Baby Unit and Maternity Wards.

There is a second tier of Paediatric medical staff to provide support.

The Paediatric Service:

The ward is a 24 bed Paediatric Ward for Medical as well as Surgical in-patients. In addition, the ward works as an 11 bed Day Assessment and Day Case unit. These wards are a significant distance from the Neonatal Unit and it is planned to locate these services within the Maternity Wing.

There is a Modern Paediatric Outpatient department with good play facilities.

The Paediatric Department offers a sub-speciality service in a number of areas. These include Paediatric Gastroenterology, Allergy and Neonatology and Respiratory Paediatrics. In addition the department has a long history of hosting joint clinics with Tertiary Specialists from outside the Trust.

Medical students from are attached to the Department.

There is strong commitment to both Undergraduate and Postgraduate Medical Education. The Consultant staff lead regular teaching of medical students, as well as junior medical staff, nurses midwives and other staff.

Secretarial support is available. Suitable office accommodation will be provided.

Clinical Audit sessions are held once a month.

The Maternity & Neonatal Services@

The maternity service is housed in a separate wing and has ante and post natal beds, as well as the delivery suite. The Neonatal Unit is situated on the floor immediately above the Delivery Suite. There are about 3500 deliveries a year on site. There is a well equipped modern Neonatal Unit with 3 intensive care cots & 17 Special Care Cots. The Neonatal Service is undergoing a period of modernisation to operate as the sole Level 2 Neonatal unit. This requires an increase in medical and nursing staff as well as investment in equipment. For many years the department has sought to train and employ Advanced Neonatal Nurse Practitioners. Our client plans to continue this programme as well as to employ a number of Clinical Fellows to fulfil service needs until a full 24 hour tier of ANNPs are in place. These proposals have the full support of the Trent Perinatal Network.
All Consultants cover both Neonatology and General Paediatrics when on-call. In addition two Consultants lead the neonatal service, both from a Clinical Governance point of view and on a day to day basis.
The Obstetric Service has 6 consultant Obstetricians, two of whom provide out-patient services in Grantham. In addition there is a further Consultant who works solely as a Gynaecologist. Junior staff rotate to nearby Hospitals.

There are five Consultant Paediatricians, two with a special interest in Neonatology. Middle grade cover is provided by 5 SpRs, one staff grade & one Teaching Fellow doctor. First level Paediatric cover is also provided by 8 ST1/2 working a full shift. Neonatal Cover is provided by 4 other Neonatal Fellows and ANNP’s, a further 2 FY1 doctors complete the staffing complement.

As a Neonatal Fellow you will be responsible for resuscitation of newborn infants on the labour suite, and will attend patients on the neonatal unit, transitional care and on the post natal wards. You will also attend the paediatric education teaching programme.

You will be on a 1in 6 rota along with ANNP’s, working on average no more than 40hours. The Neonatal Fellow will be on call for daytime as well as evening and weekend sessions for Neonates. You will be contracted as a Speciality Doctor (Fixed Term) and paid 10 basic PA’s, including provision of 1 PA weekly for your own CPD

Candidates must have MBBCh and current full GMC registration. Applicants must have good communication skills and be able to work within a multi disciplinary team. Previous experience of working in Paediatrics is essential.
